By joining Gaga activity you hereby declare that you are in good health and shape, and understand the nature of Gaga activity and to the best of your knowledge, you have no medical/health disability that could harm or endanger you during the classes and workshops and you are fit to participate in strenuous exercise and Gaga activity.

You fully accept and assume all responsibility for any injury or damage that may result from your participation in the Gaga class and any Gaga activity. You release and hold harmless Gaga Movement Ltd., and its employees and teachers, with respect to any or all injury or damages whether new or as a result of a change to an existing medical problem arising from the activity to the fullest extent permitted by law.

If there is any health problem or disability, you pledge to inform the teachers orally about this problem before starting any class. By joining Gaga activity you hereby declare that you can take part in Gaga activity without risk, considering your medical problem, and you are aware your participation in Gaga activity is enabled according to your declaration. You hereby remove all responsibility from Gaga Movement Ltd. and/or its employees and/or teachers for damages that may arise from your participation in classes and workshops and are related to your medical disability to the fullest extent permitted by law.

I am the guardian of the minors participating in the activity and confirm the health statement above on their behalf.
